چکیده

PERFUSION DURING OPEN-HEART SURGERY Since the description of the Azygos factor by Andreasen and Watson in 1951, perfusion criteria have been much debated. Early investigators fluctuated between a high and low flow rates and the debate continues today. 2 · 3 ' •· 5 '" Hypothermia has been both widely acclaimed and generally repudiated. 7 • 8 Anoxic arrest of the heart' 0 ' n. z, 12 versus coronary perfusion" is yet another question of intense debate. This latter question takes on even more significance when confronted with an already severely damaged myocardium as seen in the candidate for coronary artery bypass graft surgery. Vigorous hemodilution is acclaimed by many, others advocate the maintenance of high hematocrit to avoid sacrificing oxygen carrying capacity. In an attempt to resolve some of these questions a critical review of our perfusion data during a 22 month period was undertaken.

Surgical Anatomy of the Coronary Artery Distribution

DURING the initial phases in the development of open-heart surgery, problems related to the perfusion procedure were common causes of morbidity and mortality. With improved perfusion technics and increased knowledge of the physiology of cardiopulmonary bypass, such problems have become less frequent. Now that the perfusion procedure itself is accomplished with considerable safety, the surgical ...

Mechanical Bleeding Complications During Heart Surgery

Potential for mechanical bleeding complications is present in all forms of heart surgery, both closed and open. Successful management of such complications often is accompanied best by the rapid institution, or re-institution, of cardiopulmonary bypass, and in some instances it cannot be accomplished without the use of cardiopulmonary bypass. Therefore, the extracorporeal perfusion technologist...

The effect of varying inspiratory: expiratory ratios on gas exchange during anaesthesia for open-heart surgery.

Measurements of physiological deadspace/tidal volume ratio (VD/VT per cent) and alveolar-arterial oxygen tension difference (A-aPo2) were made during mechanical ventilation in patients undergoing open-heart surgery. The inspiratory: expiratory time ratios studied were 0.5:2.5 and 1.5:1.5 before perfusion and 0.5:2.5, 1:2 and 1.5:1.5 after perfusion. Pre-perfusion there were no significant diffe...

Effect of Instructional Videos on Postoperative Respiratory Function in Patients Undergoing Off-Pump Open Heart Surgery

Background: Respiratory function in patients undergoing open heart surgery is disrupted after surgery.  Patient education on managing complications can reduce occurrence or severity of them. A variety of educational tools has been introduced, but there are controversies about the most appropriate tools. Aim: Determine the effect of instructional videos on respiratory function of the patients af...
